The aim of this project is to ensure that people’s conversations with local services become more person-centred, to enable people to think and act on their own wellbeing. As well as training for staff we have identified other elements of work that will support person-centred approaches:

  • Personal health budgets
  • Patient activation measures – these are a measure of a person’s skills, confidence and knowledge to manage their own health.
  • Wellbeing planning - our goal is to enable people to manage their own wellbeing by supporting and encouraging them to write their own plans for care.
